I understand I am responsible for reading all ASOD communications so that I am aware of important information regarding performances and the studio.*

I understand no refunds or cancellations for recital costumes can be made after November 1, 2021.*

I understand tuition is due the first of every month and a late fee of $20 will be applied if tuition is not paid by the 6th of the month.*

I authorize Amy's School of Dance to take photos and/or videos of my children to use for marketing and educational purposes. *
